As we ventured into 2024, one of the most transformative trends in the biopharmaceutical industry is the emergence of innovative treatments for obesity, particularly the use of GLP-1 and similar drugs. These treatments not only promise to help individuals achieve their desired weight but also represent a significant economic opportunity. Driven by a society increasingly influenced by social media and the pursuit of idealized health and beauty standards, these drugs are poised to become a mega-trend, akin to the rise of AI.

In this blog post, we will explore the current obesity crisis, the impact of unrealistic societal standards, and the potential of these groundbreaking treatments to address these issues. We will also highlight the companies at the forefront of this biopharmaceutical revolution, poised to reap substantial benefits from this burgeoning trend. By examining both the societal implications and the economic prospects, we aim to provide a comprehensive outlook on how these innovations could reshape our approach to health and wellness, offering new hope and significant opportunities in the years ahead.

The Current Situation of Obesity

Obesity has reached epidemic proportions in industrialized nations, posing significant health risks including diabetes, cardiovascular diseases, and certain cancers. Obesity rates have been rising steadily over the past few decades. In the United States, approximately 42% of adults were obese by 2020. Similar trends are seen in other industrialized nations due to changes in dietary habits, reduced physical activity, and other socio-economic factors.

According to the World Health Organization (WHO), projections for the future are alarming. By 2035, nearly 50% of the global population is expected to be overweight or obese if current trends continue. This surge underscores the urgent need for effective treatments and preventive measures. The economic impact is also significant, with obesity-related healthcare costs expected to soar, further straining public health systems.

Companies Working on GLP-1 or Similar Treatments

Several companies are at the forefront of developing innovative treatments for obesity, focusing on GLP-1 agonists and other novel mechanisms. These companies aim to provide more effective and sustainable weight loss solutions compared to traditional methods.

Biohaven Therapeutics (BHVN)

Biohaven is developing Taldefgrobep alfa, a drug targeting myostatin to reduce fat while building muscle. Currently, in Phase 3 trials for spinal muscular atrophy, Taldefgrobep alfa could also be prescribed off-label for obesity, potentially offering a dual benefit of fat reduction and muscle preservation. This approach addresses a common issue with current GLP-1 treatments, which often lead to muscle loss along with fat.

Financially, Biohaven‘s stock is currently trading at $41.21 as of July 26, 2024, down about 34% from its 52-week high of $62.21.

Altimmune (ALT)

Altimmune’s pemvidutide is a GLP-1/glucagon dual receptor agonist that has shown significant promise in clinical trials. In a Phase 2 study, pemvidutide led to an average weight loss of 15.6%, outperforming other GLP-1 treatments like Eli Lilly’s tirzepatide. Pemvidutide also demonstrated better tolerability and additional benefits in reducing LDL cholesterol levels. With these results, Altimmune is advancing to Phase 2b trials, positioning itself as a strong contender in the obesity treatment market.

Altimmune‘s stock is trading at $6.75, down approximately 39% from its 52-week high of $11.10. This significant decrease suggests potential for recovery as their clinical trials progress.

Amgen (AMGN)

Amgen is developing two anti-obesity drugs, AMG 133 and AMG 786. AMG 786, an oral drug, offers a significant advantage over injectable GLP-1 treatments, potentially improving patient compliance. AMG 133, another promising candidate, may require only monthly dosing, further enhancing convenience. Amgen’s robust financial backing and extensive R&D capabilities make it a formidable player in this field.

Amgen‘s stock price is approximately $334.85 as of July 26, 2024, about 1.38% down from its 52-week high of $338.98. It has a market capitalization of over $179 billion, reflecting strong financial stability and the ability to invest heavily in R&D.

Viking Therapeutics (VKTX)

Viking’s GLP-1 receptor agonist VK2735 has shown promising results in early trials. The oral administration of VK2735 could offer a preferable alternative to injectables, with good tolerability and effective weight loss. Viking is progressing to Phase 2 trials, aiming to solidify its position in the obesity treatment landscape. Additionally, Viking’s VK2809, targeting NASH, has shown significant efficacy in reducing liver fat, highlighting the company’s broader therapeutic potential.

Viking‘s stock is trading at $65.98, up significantly from earlier in the year but down about 34% from its 52-week high of $99.70. The company’s market capitalization is approximately $7.31 billion. The promising clinical trial results and strategic focus on metabolic diseases position Viking well for future growth.

Fractyl Health

Fractyl is pioneering a gene therapy approach with its Rejuva platform, designed to stimulate GLP-1 expression directly in the pancreas. This method aims for long-term metabolic benefits from a single dose, potentially revolutionizing obesity treatment. Fractyl is also advancing Revita, an endoscopic procedure targeting the gut’s role in metabolic diseases. Both approaches underscore Fractyl’s innovative strategies to address obesity and related conditions.

Fractyl’s financials are more private as it prepares for its IPO. However, the company has secured substantial funding, with a recent cash position of $121.4 million, sufficient to fund operations through key development milestones.

What Sets These Companies Apart from Novo Nordisk

Novo Nordisk, a current leader in GLP-1 treatments with drugs like Ozempic and Wegovy, has set a high benchmark in obesity management. However, the companies mentioned above are distinguishing themselves through various innovative approaches:

  • Biohaven’s Focus on Muscle Preservation: By targeting myostatin, Biohaven aims to reduce fat while preserving muscle, addressing a key limitation of existing GLP-1 treatments.
  • Altimmune’s Dual Receptor Agonist: Pemvidutide’s combination of GLP-1 and glucagon receptor activation offers a dual mechanism for weight loss, potentially enhancing efficacy and tolerability.
  • Amgen’s Oral and Less Frequent Dosing: The development of oral treatments like AMG 786 and monthly dosing options like AMG 133 could significantly improve patient adherence and convenience.
  • Viking’s Oral Administration and Broader Therapeutic Targets: Viking’s oral GLP-1 agonists and their work on NASH demonstrate a comprehensive approach to metabolic diseases.
  • Fractyl’s Gene Therapy and Gut-Targeted Treatments: Fractyl’s innovative gene therapy and endoscopic procedures highlight novel mechanisms aiming for long-term metabolic improvements.
